GCMS Middle School Registration 2014

GCMS Middle School Registration
2014-2015
Table 1:
Welcome to registration.  At this table you will receive the letter you are reading and your students demographic information sheet.  Please read over and correct all information on the demographic page.  This information is essential for our communication throughout the school year.  ALL phone numbers and emails need to be accurate.  If you have a change in contact information it is important you communicate those changes as the year progresses.
Table 2: (required)
ALL students should pick up and complete the student handbook form, photo release form, internet use form, and concussion form (all who participate in extra curriculars).  All students will get their agenda books at this station.  (all items will be filled out in the cafeteria)
Table 3: (optional)
This table is full of information and opportunity.  Much of what you see here is also available online.  Take what pertains to you.  The medication authorization form is important for anyone who intends to have their child take either prescription or over the counter meds.  It is important to note on this sheet that if it is a prescription med it must be signed by the prescribing doctor before distribution can occur at school.  Forms to apply for free and reduced lunch and fee waivers can be picked up here.  Anyone who qualifies for free or reduced lunch also qualifies for a fee waiver and must fill out that form!
Table 4:
PTA will be here to encourage participation and support for this great organization that provides for students K-12 at GCMS.
Cafeteria:
Please use the cafeteria as your location to fill out all documents.  This will expedite the process of registration for everyone!!!
Table 5:
All forms need to be turned into their appropriate bins.  Some are color coded to make it easier, but please be sure to place the items in the correct place to ensure proper filing and handling.
Table 6:
Parents- Please turn in your free and reduced lunch forms and or fee waivers to Mr. Darnell for approval.  Students (if present) and parents will also pick up their students schedule which will have their locker assignments and combination on them.                                             (turn over!!!)
Office:
Registration fees, activity passes, yearbooks, and supply coupons will all be taken care of in the office.  All students must complete the process in the office, even if fees are waived.
